THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
This Notice of Privacy Practices (the “Notice”) describes how protected health information about you may be used and disclosed and how you can obtain access to that information. It also describes your rights and our legal obligations with respect to your protected health information.
For purposes of this Notice, “we,” “us,” and “our” refer to the professional medical corporations, provider groups, and individual providers that provide clinical services in connection with Reneu Wellness Club (collectively, the “Clinicians”), including their licensed healthcare providers and workforce members. Reneu Wellness Club, LLC may provide administrative, technology, and management support services to these Clinicians.
OUR OBLIGATIONS
We are required by law to:
• Maintain the privacy and security of your protected health information;
• Provide you with this Notice describing their legal duties and privacy practices;
• Notify affected individuals following a breach of unsecured protected health information, as required by law; and
• Follow the terms of the Notice that is currently in effect.
HOW WE MAY USE AND DISCLOSE MEDICAL INFORMATION ABOUT YOU
The categories below describe the ways the Clinicians typically use and disclose protected health information without further authorization from you. Not every use or disclosure in a category is listed.
For Treatment: We may use and disclose medical information to provide, coordinate, or manage your healthcare and related services. This includes disclosures to physicians, nurses, pharmacies, laboratories, and other healthcare providers involved in your care.
For Payment: We may use and disclose medical information to bill and collect payment for healthcare services. This may include disclosures to health plans for prior authorization, eligibility, and payment purposes. If you pay in full out-of-pocket and request that we not disclose information about that service to your health plan, we will honor that request as required by law.
For Health Care Operations: We may use and disclose medical information for operations necessary to run our practices, including quality assessment, credentialing, licensing, auditing, compliance, and business management.
Health Information Exchanges: We may share medical information through secure electronic health information exchanges with other providers involved in your care. You may opt out of participation by contacting us in writing at the address listed below.
Quality Assurance and Utilization Review: We may use or disclose information to assess quality of care, evaluate services, and ensure appropriate treatment levels.
Credentialing and Peer Review: Medical information may be used to evaluate provider qualifications, performance, and professional conduct.
Treatment Alternatives and Health-Related Information: We may use and disclose medical information to inform you about treatment alternatives or health-related services that may be relevant to your condition.
Appointment Reminders: We may contact you to remind you of appointments or follow-up care.
Vendors and Business Associates: The Clinicians may disclose medical information to third parties (referred to as “business associates” under HIPAA) that perform services on their behalf (such as Reneu Wellness Club LLC and other entities that provide services such as billing, technology, or legal services). These parties are required by law or contract to safeguard your information.
Individuals Involved in Your Care: We may disclose information to family members or others involved in your care or payment, as permitted by law or with your agreement.
As Required by Law: We may disclose medical information when required by federal, state, or local law.
Other Permitted Uses: We may use or disclose information as permitted by law for public health activities, health oversight, research, law enforcement, judicial proceedings, workers’ compensation, military activities, coroners or medical examiners, or to prevent a serious threat to health or safety.

Clinicians may communicate with you through methods you authorize, such as email, text message, telephone, or through a Reneu-supported patient portal or platform.

ELECTRONIC DISCLOSURES
Under the law of certain states, we are required to provide notice to you if your medical information is subject to electronic disclosure. This Notice serves as general notice that we may disclose your medical information electronically for treatment, payment, or health care operations or as otherwise authorized or required by state or federal law.
OTHER USES OF MEDICAL INFORMATION
Authorizations
Uses and disclosures not described above will be made only with your written authorization. You may revoke an authorization in writing at any time, except to the extent action has already been taken.
Psychotherapy Notes, Marketing, and Sale of Information
Most uses and disclosures of psychotherapy notes, marketing uses of protected health information, and disclosures constituting a sale of medical information require your written authorization. The Clinicians do not anticipate selling medical information.
YOUR RIGHTS REGARDING MEDICAL INFORMATION
You have the right to:
Inspect and copy your medical records. You have the right to inspect and/or copy your medical information maintained in a designated record set, which generally includes medical and billing records. To do so, submit a written request to privacy@renuwellnessclub.com. We may charge a fee for copying, mailing, or supplies, consistent with state law. If your records are electronic and readily producible, we will provide them in your requested format. In limited circumstances, we may deny your request. If denied, you may request a review by another licensed health care professional, and we will abide by the outcome.
Request amendments to your medical information. If you believe your medical information is incorrect or incomplete, you may request an amendment at any time. Submit your request in writing to privacy@renuwellnessclub.com with the reason for the amendment. We may deny requests that are not in writing, unsupported, or pertain to information that is accurate, complete, not created by us, or not part of the designated record set. Denials will be provided in writing.
Receive an accounting of disclosures. You have the right to request a list of disclosures of your medical information, except those for Treatment, Payment, Health Care Operations, or pursuant to authorization. Requests must be in writing to privacy@renuwellnessclub.com, specifying the time period (up to six years) and desired format.
Request restrictions on certain uses or disclosures. You have the right to request a restriction or limitation on the medical information we use or disclose about you for Treatment, Payment, or Health Care Operations. You also have the right to request a restriction or limitation on the medical information we disclose about you to someone who is involved in your care or the payment for your care, like a family member or friend. Except as specifically described below in this Notice, we are not required to agree to your request for a restriction or limitation. If we do agree, we will comply with your request unless the information is needed to provide emergency treatment. In addition, there are certain situations where we won’t be able to agree to your request, such as when we are required by law to use or disclose your medical information. To request restrictions, you must make your request in writing to privacy@reneuwellnessclub.com. You should be aware that such restrictions may have unintended consequences, particularly if other providers need to know that information (such as a pharmacy filling a prescription). It will be your obligation to notify any such other providers of this restriction.
Request confidential communications. You have the right to request that we communicate with you about medical matters in a certain way or at a certain location. For example, you can ask that we only contact you through a personal email address and not at work or, conversely, only at work and not a personal email address. To request such confidential communications, you must make your request in writing to privacy@reneuwellnessclub.com. We will not ask the reason for your request, and we will use our best efforts to accommodate all reasonable requests, but there are some requests with which we will not be able comply. Your request must specify how and where you wish to be contacted.
Obtain a paper or electronic copy of this Notice. You have the right to a paper copy of this Notice. You may ask us to give you a copy of this Notice at any time. To obtain a copy of this Notice, you must make your request in writing to privacy@reneuwellnessclub.com.
Receive notification of a breach, if applicable. In certain instances, we may be obligated to notify you (and potentially other parties) if we become aware that your medical information has been improperly disclosed or otherwise subject to a “breach” as defined in and/or required by applicable law.
Requests must be submitted in writing to privacy@reneuwellnessclub.com. Fees for certain requests may apply as permitted by law.
CHANGES TO THIS NOTICE
The Clinicians reserve the right to change this Notice at any time. The revised Notice will apply to information already maintained as well as information received in the future. Updated versions will be posted on the applicable website or made available upon request.
COMPLAINTS
If you believe your privacy rights have been violated, you may file a complaint with the applicable Clinician by contacting privacy@reneuwellnessclub.com. Filing a complaint will not result in retaliation. You may also file a complaint with the U.S. Department of Health and Human Services.
